Validación y curva de aprendizaje del mapeo linfático y linfadenectomía del ganglio centinela en mujeres con cáncer de mama temprano

RESUMEN
El mapeo linfático (ML) y la linfadenectomía del ganglio centinela (LGC) surgen como alternativa para identificar el estatus axilar y así evitar el vaciamiento ganglionar. Antes de abandonar la disección axilar se debe validar el método hasta alcanzar el hallazgo del ganglio centinela (GC) en más del 90% y falsos negativos en menos del 10%. El objetivo de este estudio es evaluar la validación del método de ML y LGC así como la curva de aprendizaje requerida en mujeres mexicanas con cáncer de mama temprano. Se revisaron retrospectivamente 77 casos seleccionados de mujeres con cáncer de mama invasor sin ganglios palpables, etapas I y II de la UICC 2002, vistas en el servicio de Tumores de Mama del Hospital de Oncología del Centro Médico Nacional Siglo XXI en un periodo del 1° de marzo de 2002 al 31 de diciembre de 2004, sometidas a ML y LGC con técnica de azul patente sola o azul patente y radioguiada combinada, seguidas de disección axilar en todos los casos. En 92% (72 de 77 casos) se encontró el ganglio centinela (GC), el 80% de los casos no hallados (4 de 5) fueron buscados con técnica de azul patente sola y durante el primer año del estudio. De los 72 casos hallados en igual número de mujeres el promedio de edad fue de 56.2 años (28-82), el tamaño promedio del tumor primario 26 mm (8-45), etapa clínica I 12% (9 de 72), etapa clínica IIA 78% (56 de 72), etapa clínica IIB 10% (7 de 72). El GC fue buscado con técnica de colorante en 54% (39 de 72) y técnica combinada en 46% (33 de 72), en 44% (32 de 72) hubo ganglio(s) positivo(s), en 59.3% el GC fue el único ganglio positivo (19 de 32). Hubo un porcentaje de falsos negativos (GC negativo y algún ganglio de la disección axilar positivo) de 9.7 (7 de 72). De estos 7 pacientes 5 fueron con técnica de colorante solo y todos ocurrieron durante el primer año del estudio. Para establecer la curva de aprendizaje evaluamos el porcentaje de hallazgo del GC y de falsos negativos por cirujano, así los cirujanos A, B, C y D representan a cirujanos oncólogos con más de dos años en el servicio, la letra E representa cirujanos rotatorios. Los cirujanos A, B, C y D superaron individualmente el hallazgo de GC en más del 90%, pero los cirujanos A y B tienen más de 10% de falsos negativos, con 16 casos y requieren 5 más para alcanzar la cifra de menos de 10% de falsos negativos; los cirujanos C y D con 28 y 8 casos, respectivamente, tienen cifras de validación aceptadas. El cirujano D con pocos casos es el más nuevo del servicio y tiene la ventaja de haber visto el método durante la residencia. El grupo de cirujanos E tiene hallazgo de 82% de GC y falsos negativos de 28%, cifras inaceptables en la validación. Con los resultados se concluye que la validación del MG y LGC es un método indispensable antes de abandonar la disección axilar; la técnica combinada, el tiempo, la experiencia y el número de casos influyen en los resultados; así entonces, para cirujanos con experiencia en cirugía mamaria el número recomendado es más de 20 casos, aunque quienes tienen contacto con la técnica durante el entrenamiento el número puede ser menor.
