Eficacia y seguridad de la budesonida en el tratamiento de la rinitis alérgica en niños: Revisión sistemática con metaanálisis

RESUMEN
Justificación: La rinitis alérgica (RA) es la patología crónica más frecuente en la población pediátrica. Posee un impacto significativo en la calidad de vida (QoL), interacciones sociales y condiciones comórbidas, produciendo gastos directos, indirectos e intangibles altamente significativos. Se considera importante evaluar en forma sistemática y exhaustiva la evidencia disponible con el objetivo de identificar en forma no sesgada la eficacia y seguridad de la budesonida para el tratamiento de esta patología.
Diseño del estudio: Revisión sistemática con metaanálisis en la que se incluyeron sólo ensayos clínicos controlados, aleatorizados, efectuados en menores de 18 años, con rinitis alérgica y que hubieran recibido al menos en una de las ramas de tratamiento budesonida por vía nasal, comparado con placebo o algún otro antihistamínico. Se consideraron como variables de desenlace el porcentaje de pacientes que recibieron una mejoría total o casi total de la sintomatología nasal, la reducción en el puntaje de síntomas nasales, la mejoría en el puntaje de la escala de calidad de vida (PRQLQ) y la mejoría observada en el flujo pico a nivel nasal (PNIF), así como el cambio en el porcentaje de eosinófilos en moco nasal. Desde el punto de vista de seguridad se analizaron el porcentaje de eventos adversos observados en cada uno de los grupos de tratamiento, el potencial impacto en la ganancia de talla (cm), así como los valores de cortisol urinario ajustados por niveles de creatinina (cortisol urinario/creatinina). Se efectuó el cálculo de razón de riesgos (RR) en el caso de desenlaces binarios y diferencia de promedios ponderada (WMD) en el caso de desenlaces continuos. Para todos los estimados se efectuó cálculo del intervalo de confianza al 95% (IC95%). Se efectuó en todos los casos prueba de heterogeneidad utilizando prueba de Chi cuadrada de Pearson, con un valor de p ‹ 0.05 como sinónimo de significancia.
Resultados: Se identificó un efecto positivo a favor de budesonida en relación con la mejoría total o casi total de la sintomatología nasal [RR 1.66 (IC95% 1.33 a 2.07), p ‹ 0.0001], la reducción en el puntaje de síntomas nasales [WMD -0.67 (IC95% -0.69 a -0.66), p ‹ 0.0001], la mejoría en el puntaje de la escala de calidad de vida o PRQLQ [WMD 0.16 (IC95% 0.14 a 0.18), p ‹ 0.01], y la mejoría observada en el flujo pico a nivel nasal (PNIF) [WMD 21.4 (IC95% 19.6 a 23.2), p ‹ 0.0001]. Desde el punto de vista de seguridad no se observaron diferencias significativas en relación con el porcentaje de eventos adversos observados en cada uno de los grupos [RR 1.05 (IC95% 0.8 a 1.3), p = NS], ni en la ganancia de talla (cm) [WMD -0.02 (IC95% -0.05 a 0.01), p = NS].
Conclusiones: Se recomienda budesonida como una alternativa eficaz y segura para el tratamiento del menor de 18 años con rinitis alérgica al controlar casi en forma total, en la mayoría de los pacientes los síntomas presentes, reducir los puntajes de las escalas de evaluación de los síntomas nasales, mejorar la calidad de vida así como el flujo nasal de aire, sin producir eventos adversos en forma significativa, ni efectos deletéreos en el crecimiento pondoestatural.
