Invasión linfovascular y p53 mutado como factores predictores de metástasis ganglionares axilares en cáncer de mama

RESUMEN
Introducción: Recientemente se han estudiado factores histológicos en el tumor primario de mama que permitan predecir el estado de los ganglios axilares. El objetivo de nuestro estudio fue analizar los factores histológicos clásicos del tumor y genéticos con estudio del gen supresor tumoral p53 como probables factores predictores de afección ganglionar metastásica en la axila por cáncer de mama.
Materiales y métodos: Este estudio analítico tipo Expost-Factor incluyó a 100 pacientes con diagnóstico de adenocarcinoma de mama, a quienes se les realizó una cirugía mamaria con disección de axila. El análisis histopatológico correlacionó el valor predictivo de la invasión linfovascular del tumor y la mutación presente del gen p53, así como la probabilidad de tener metástasis axilares.
Resultados: El promedio de edad fue de 54.5 años. De 30 pacientes con invasión linfática, 24 (80%) presentaron metástasis a ganglios axilares; 16 de 70 casos sin invasión linfática tuvieron metástasis axilares (p = 0.000008). La invasión vascular solamente se presentó en 12 pacientes, sin diferencia estadística significativa para afección metastásica axilar (p = 0.16). En 18 pacientes hubo mutación del gen p53; de ellos, 15 presentaron metástasis axilares (83%), diferencia estadísticamente significativa (p = 0.0025). Hubo valor predictivo positivo para infiltración ganglionar axilar del 83.3%. En los tumores sin mutación del gen, la correlación con la presencia de las metástasis axilares no fue predictiva.
Conclusiones: La invasión linfática y vascular asociada con el tumor primario son factores pronósticos adversos asociados con las metástasis ganglionares axilares. El gen p53 mutado en el tumor primario es un valor predictivo positivo para las metástasis axilares.
