ABSTRACT

The early dysfunction of the renal graft (DTIR) implies the possibility of loss of the graft, representing a this one surgical medical urgency; the medical causes must be considered, most frequent are: The acute rejection, acute tubular necrosis and toxicity by medicines. On the other hand, linfocele, vascular trombosis, arterial estenosis and urinary flight represent the surgical complications that, most frequently, usually condition to DTIR. Meticulous hemostasis intraoperating, monitorization and hemodynamics control prevent hemorrhagic complications, like hematoma subcapsular renal that can jeopardize the function of the renal graft. In this paper it is informed of a case of formation and early expansion of hematoma subcapsular renal (renal compartamental syndrome), associated with trombocitopenia and severe primary dysfunction, with recovery of the renal function surgical posdrenaje of hematoma, and subsequent formation of hematoma parenquimatoso left frontal and frontal syndrome, that displayed descompresiva craniotomy.
